Missed Appointment Log

(Padded in 50's) -- This form has the usual headings to include the patient's name, phone #, etc. but also headings to make special notes regarding a "Connector" that may be helpful in re-scheduling, and whether or not the patient may be eligible for a "Make-Up" appointment. We keep this log current daily, and transfer the names over to a "Master" log once per week. It's a great tool to help boost your volume and retention.
